“Special Needs” Program for Individual Patients and Families
Perhaps the most compelling and concrete examples of how Big "R" & Smokin' Joe's Hospice Benefit resources are used to make a difference in people's lives fall under the category of "Special Needs." Big "R" & Smokin' Joe's Hospice Benefit makes grants to fund extraordinary expenses that lie outside the realm of hospice care but are nevertheless critically important to the patient's comfort or peace of mind.
The examples are many. For the patient dying in the middle of summer in a stifling apartment,
Big "R" & Smokin' Joe's Hospice Benefit provides funds for a window air conditioner. For another patient,
Big "R" & Smokin' Joe's Hospice Benefit makes it possible for them to get
new glasses, a small tv, heat for the winter....

Big "R" & Smokin' Joe's Hospice Benefit will primarily fund extraordinary needs of indigent hospice patients through donations contributed to the Big "R" & Smokin' Joe's Hospice Benefit.
The purpose is to enrich the lives of these patient's and their families by relieving distress/hardship.
Funding for extraordinary needs beyond what a hospice would normally provide is available to all hospice patients who qualify under the guidelines. Funds will not be allocated for patient's needs that should be met by the hospice under the reimbursement system of Medicare, Medicaid or third party payment.

Process for Special Needs Request to
the Big "R" & Smokin' Joe's Hospice Benefit
1) The emailed/USPS mailed application is received in the office of Big "R" & Smokin' Joe's Hospice Benefit once the application is complete, including supporting documents and required signatures from the person recommending this request, Team Manager, Sr. Manager or other professional not-for-profit staff. The information is given to the Big "R" & Smokin' Joe's Hospice Benefit Board of Directors.
2) The patient's financial situation is assessed to determine that the request falls within Big "R" & Smokin' Joe's Hospice Benefit guidelines.
3) A decision is made after the complete application is received to grant none, some or the entire request amount. A member of the Big "R" & Smokin' Joe's Hospice Benefit Board of Directors signs the form and presents a check, the item requested or a gift card/gift certificate.
4) The person who made the request on behalf of a patient (usually a hospice worker, social worker or chaplain) is contacted either by phone or mail to let them know the Big "R" & Smokin' Joe's Hospice Benefit Board of Directors final decision.
5) A vendor check or gift card may be sent to the hospice staff member who requested food, clothes, fuel, etc. for the patient. The vendor check will then be taken by the hospice staff member to purchase the item or gift cards for the patient. Gift cards or Gift Certificates (specified local stores, such as Carrows, Witbecks, not pre paid Visa/Mastercard) may be given directly to the patient/caretaker by the hospice staff member.
